Our Association and our nation have no more valuable resource than our children. The more carefully we nurture our children, the better the chances for a bright future for all of us. It is with this thought that in 1969, our Association founded the “William Penn Fraternal Association Scholarship Foundation, Inc”.

The William Penn Association Foundation, Inc. is a non-profit corporation. Its purpose is to generate donations from the William Penn Association membership, business relations and general public. These funds are then disbursed as contributions for charitable, fraternal, religious, cultural, educational or humanitarian purposes within or outside the United States.

Downloadable Forms

To make things easier for everyone, we have included some of our forms in .pdf format for you to download.

These forms must be followed correctly and sent in to the home office to the address shown at the very bottom of the page.

Request for Change in Insurance Contract - This form has various sections. By downloading this from you can change your address, name, beneficiary, premium mode, and dividend options.

Branch Membership Transfer - The name of this form speaks for itself. This form gives you the ability to switch to a new branch.

Contract Service Request - This form has various uses, including withdrawal from non-qualified annuities, cash surrenders, assignment of ownership, request duplicate contract, and a miscellaneous section.

Electronic Transfer (Premium Payments) - By filling out this form you can automatically have your premium payments taken from a bank account.
